Download the PDF, complete the required information, stamp (if applicable), and submit the confirmation to FINconsult.
How to use the confirmation
- Download the PDF confirmation.
- Fill in: subscriber name, address, phone number, email, and contact person.
- Enter full name (CR/OR/CRT) and license number.
- Enter place and date, stamp (if applicable), and sign.
- Submit the confirmation to FINconsult (email or fax).

Note: For one copy of the journal, the confirmation is issued for one individual only; for multiple copies, a separate confirmation must be submitted for each subscriber.
